almost all firearms are made to NOT have an OOB malfunction. So how does this Sig fire out of battery regardless of the slide face angle?? I'm not understanding how that occurs. This indicates that ALL Sig pistols must be suspect of the firing pin being allowed to go forward with inertia to produce a slam fire such as this no?

correction. The Chicago Police Dept. has removed these models
from their approved weapons category for officers purchasing duty
related pistols and will not change their policy until the design
change has been remedied.

After reviewing these circumstances, it is the recommendation of
your Safety Committee along with the approval of the National
Board to direct our members NOT to purchase any of those models
stated above until Sig-Arms changes their design to encompass the
"Chicago Modification", and offer to modify ALL of those model
weapons currently owned by our members.

That bulletin was from May of 1997. It seems like Sig appeased the Chicago PD with the modification, but that was truly wasn't the root cause of the out of battery situation.
